The City of Boise provides Basic Life and Accidental Death & Dismemberment (AD&D) Insurance including limited dependent coverage, to all Fire Fighters who are regularly scheduled to work 20 or more hours per week.

Basic Term Life

The city pays the cost of coverage for Basic Life and AD&D at one times base salary, up to a maximum of $125,000, to all Fire Fighters working at least 20 hours per week.

Basic Dependent Term Life

The city also pay the cost of Basic Dependent Life. A spouse is covered at $5,000 and children at $2,500.

Optional Term Life

Fire union employees may elect to purchase Optional Term Life Insurance from one to four times annual base salary, up to a maximum of $500,000.

New employees may purchase up to two times their annual base salary without having to complete a Medical History Statement. Bi-weekly rates are based on the employee's age.

Enrollment

Employees have 30 days from their date of employment to elect any additional coverage. After the initial 30-day enrollment period, employees may enroll but will be required to complete a Medical History Statement which is subject to approval.

Cancellation

Employees may cancel any additional coverage by sending an e-mail to payrollandbenefits@cityofboise.org, or written notice to Human Resources indicating they no longer wish to participate.

NOTE: Employees may not decline Basic Life and AD&D Insurance coverage.
